Course Reserves Submission Form

Please use this form to place materials (both physical and digital) on reserve for your class. We will also add the materials to Course Reserves Search.

If you are placing materials you own on reserve, please print the confirmation page that will appear after you have submitted your request. Bring the page with you to the Circulation Desk, along with the materials to put on reserve.

Instructor & Course Information

If you want to submit more than 10 items, fill out the information above to include your course information. Then email a copy of your syllabus or resource list to the reserves manager at your campus. Please ensure the option above is checked, and please include your desired loan periods for your items in your email.
